The initial phase of hospice care involves an assessment process. In this phase, we assess the patient’s health status to decide if they qualify for services within the hospice program.

Once confirmed as eligible, the patient will be entrusted to a multidisciplinary team that includes a medical director, nurses, social workers, home health aides, chaplains, volunteers, and other experts skilled in end-of-life care. This team will devise a customized plan tailored to the patient’s specific needs.

Key aspects of hospice comfort care include:

  1. Pain and Symptom Management: Medical professionals work to manage pain and other distressing symptoms, such as nausea, shortness of breath, and fatigue, to ensure the patient is as comfortable as possible.

  3. Family Support: Hospice care extends beyond the patient, offering counseling, respite care, and support to family members and caregivers.
